General guidelines for referrals

When completing an Student Alert System form, please remember...

  1. The instructor should try to contact the student first if possible.
  2. The form should include concrete, factual, objective information, not opinions or supposition.
  3. No behavioral or personality comments should be included.
  4. To include specific examples, e.g. the student has missed 5 classes, the student received a 65% on Unit 1 test.
  5. To phone Student Support Services on situations requiring immediate attention or dealing with counseling issues.
  6. To include Social Security numbers for Banner look-up.
  7. To spell the student's name accurately.
  8. The form is a referral for services.
  9. To include course CRN number.
