General guidelines for referrals
When completing an Student Alert System form, please remember...
- The instructor should try to contact the student first if possible.
- The form should include concrete, factual, objective information, not opinions or supposition.
- No behavioral or personality comments should be included.
- To include specific examples, e.g. the student has missed 5 classes, the student received a 65% on Unit 1 test.
- To phone Student Support Services on situations requiring immediate attention or dealing with counseling issues.
- To include Social Security numbers for Banner look-up.
- To spell the student's name accurately.
- The form is a referral for services.
- To include course CRN number.
